Job Description:

The Interventional Radiology Oncology Patient Care Coordinator is a member of a multi-disciplinary team with primary responsibility for direct and indirect professional nursing care of interventional radiology oncology patients. Provides clinical support services to physicians in the Interventional Radiology Practice. The Patient Care Coordinator will manage clinic and pre and post interventional procedures patients. Responsibilities include:+ Development and delivery of patient education+ Assessing patient and family needs+ Developing nursing care plans+ Implementing and evaluating the care delivered+ Cordinating follow up care for patients as recommended by the interventional radiology care team including but not limited to communication with the patient as well as the referring providers+ Assisting with monitoring diagnostic results and the patient care progress/processes as requested by providers+ Managing the Y-90 program including but not limited to assisting in the administration of the liver directed treatment+ Acts as the patient navigator throughout OHSU and other HealthCare facilities as necessaryThe Patient Care Coordinator is expected to practice in accordance with professional standars, core values, and with a commitment to service excellence. Will be expected to practice in accordance with the mission of the Dotter Department of Interventional Radiology as well as perform requirements as outlined in compliance roles and responsibilities in OHSU Hospitals and Clinics. Education+ External applicants: Baccalaureate of Science in Nursing Degree from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA)+ Internal applicants: Associate's Degree in NursingRegistrations, Certifications and/or Licenses+ Current, unencumbered Oregon State Registered Nurse LicenseExperience+ 5 years of experience in Medical/Surgical nursing and/or 2 years in Oncology nursingSkills+ Basic computer skills including the ability to send/receive email, navigate information technology associated with the position, and use Electronic Health Record information and tools+ Strong communication skills with fluency in oral and written English+ Current OHSU employees: BSN preferred+ Specialty Care Management and Interventional Radiology patient care+ Ambulatory nursing experience+ Experience working and coordinating in a multidisciplinary practice model+ Experience with Epic+ Certification in Basic Life SupportOrientation Period: 6-8 weeksOregon Health & Science University values a diverse and culturally competent workforce.
