POSITION PURPOSE: The Registered Nurse (RN) located in Warrenton
OR is employed full-time and is directly responsible to the
Administrator for ethical, professional, and adequate delivery of
health services to residents that meet or surpass regulations;
promote over-all resident well-being through direct services,
over-sight and training of health services staff; and has essential
training and compassion in the provision of care to residents with
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Supports the holistic well-being of residents at Clatsop Care
Supports the success of employees including contributing to an
environment of mutual respect and individual growth.
Functions within the scope of practice for a Registered Nurse in
the State of Oregon.
The RN shall participate in the development and implementation of
any facility policies that affect the delivery of health care
Delegates to appropriate staff nursing tasks in accordance with the
rules of the Oregon State Board of Nursing and the Oregon
Administrative Rules for Assisted Living.
Ensures that all medications and treatments are given in accordance
with Physician Orders and coordinates the medication delivery
Audits MAR and TAR records at least monthly.
Monitors resident vitals - prepares monthly documentation
Assures that quarterly recaps are accomplished.
Meets with Consultant Pharmacist and facilitates actions in
accordance with recommendations. In the event the consultant
pharmacists is unavailable, reviews all medications and treatment
at least every 90 days.
Reviews and forwards pharmacy recommendations to resident
physicians and ensures implementation or appropriate documentation
Consults on residents with behavioral symptoms before psychoactive
medications or restraints are requested by the facility. Implements
PRN parameter sheets, and interventions on any psychotropic
Participates in quality assurance program providing audits and
attending meetings as assigned by Administrator.
Participates with the facility administrator and other department
directors in development and maintenance of practices and
procedures that promote infection control, fire safety, quality
assurance and hazard reduction.
Participates in educational programs in accordance with licensure
and facility needs, including new employee orientation and

phone calls in timely manner.
Participates with the Administrator in the hiring of employees
assigned to health services.
Directs, supervises, and evaluates Medication Aides/Team
RN will give input and assistance when needed for employee
Reviews resident's individual service plan in accordance with rules
for licensure and assures services are delivered in accordance with
mutually agreed upon plan.
Participates in service planning when resident is receiving nursing
services or experiences a significant change in condition.
The RN is knowledgeable of the general health and well-being of
every resident in the facility.
Participates in coordinating off-site and on-site health services
by other providers and reviews service plan changes made as a
result of provision of on-site services.
Approves facility system for tracking controlled substances.
Assists Medication aides in the destructions of medications.
Participates in negotiated managed Risk Agreements when residents
chose activities that are contraindicated to good health and safety
practices with Facility Administrator.
Reviews resident accident and incident reports and assures that
established procedures are followed. Submits reports to
Administrator within 30 days of occurrence - monthly.
Conducts an Admission Assessment when requested by Administrator to
ensure residents are placed at appropriate levels and that the
facility has the ability to meet their need requirements.
Participates with the facility administrator in making budget
recommendations and facilitates budgetary compliance when

when a significant change in a resident's condition warrants
nursing or medical intervention. Assures that health services staff
are informed of resident change in conditions.
Participates in those committee meetings as assigned by the

Graduate of an accredited program of nursing education. Current
registration as a RN in good standing with the State of Oregon.
One year experience in working with an elderly population.
Good mental and physical health, sound judgment and high moral
standards, and a sincere desire to work with the aged and those
with limited capacity for self-care and in-particular with
residents with varying degrees of dementia. Patience and compassion
Ability to manage and supervise employees. Interest and ability in
teaching, evaluating, and assisting health care staff in improving
Capacity to develop and learn new theories, techniques and
principles and implement them.
Ability to make independent decisions.
Willingness to attend workshops and institutes in field of
Geriatrics and Dementia Care.
Adequate computer skills and ability to use electronic records
related to health care.
Ability to communicate with staff, residents, families, outside
providers and general public effectively through oral and written
