sustains a Patient-Centered Medical Home (PCMH) care delivery model
that is preferred by many practicing clinicians. Under this model,
patient treatment is coordinated through a patient's primary care
provider to ensure that they receive the necessary care when and
where they need it, and in a manner that they can understand. The
objective of the PCMH model is to maintain a centralized setting
that facilitates partnerships between individual patients and their
providers and, when appropriate, the patient's family as well. Care
is facilitated by registries, information technology, health
information exchange, and other means to assure that patients get
the indicated care in a culturally and linguistically appropriate

Qualifications:
Graduation of an accredited college
or university with a Doctor of Medicine degree. Successful
completed of an approved residencyBoard certification in specialist
or practice required. Must maintain state licensure and DEA
certificate. Current AHA BLS certificationSkills in providing
diagnostic, preventative and or therapeutic services to
patientsKnowledge of diagnosing and treating medical problems and
developing care plansAbility to educate patients, families, and
staff in an effective mannerFull COVID-19 vaccination is an
