DescriptionWill work with interdisciplinary teams at Providence ElderPlace to provide pharmaceutical care services to geriatric participants. The pharmacist will provide drug therapy management and consultative services which will include a review of participant's drug profiles, working with prescribers to optimize drug regimen, monitoring quality of pharmacy practice, and providing inservice education to Interdisciplinary Teams. The clinical pharmacist will also serve as ElderPlace's liaison to the dispensing pharmacy. Efforts will focus on optimizing drug therapy outcomes, improving drug utilization, improving patient compliance, preventing adverse drug events, and acting as a drug therapy resource to physicians and other members of the health care team.Required qualification:

• Graduate from an accredited college of pharmacy.
• PharmD
• Required upon hire: Oregon Pharmacist License
• Required within 2 years of hire National Certification from Board of Pharmacy SpecialtiesPreferred qualification:
