The Field Based Community Health Care Ambassador is a pivotal member of the Landmark Interdisciplinary Team. This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients' and caregivers' trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.

This position is full - time. Employees are required to work our normal business hours of 8:30 AM - 5:00 PM. Our office is located at 700 SE MULTNOMAH ST, STE 400, PORTLAND, OR, 97232. We offer 6 weeks of training.

Primary Responsibilities:

• Conduct home visits and / or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:
• Resource referral and connection
• Quality measure documentation, support, and follow up
• Telemedicine facilitation and set up
• Collect information regarding patient conditions using standardized checklists and templates
• Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction
• Actively participate in weekly interdisciplinary team meetings
• Reports directly to the Supervisor, Clinical Nurse

  Required Qualifications:
  • High School Diploma / GED (or higher)
  • 3+ years of experience in any of the following job categories or categories similar to: Social Worker; Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator; Healthcare Technician
  • Ability to conduct independent home visits
  • Experience with using Microsoft Office products (Microsoft Word, Microsoft Excel, and Microsoft Outlook)
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy
  • Ability to travel up to 10% of the time (Domestic)
  • Reside within Oregon (Oregon side, not Vancouver) and a 30 - 45 minute commutable distance of Portland, OR office
  • Ability to work any shift between the hours of 8:30 AM - 5:00 PM

    Preferred Qualifications:
    • Geriatric OR primary care experience, OR experience in institutional settings of care such as long - term care / skilled nursing facilities
