Job SummaryThis role provides coordination and case management for ventricular assist devices (VAD) and cardiac transplant patients in collaboration with the program medical director and referring physicians to promote optimal patient outcomes.Key Responsibilities

• Coordinate patient care and services for VAD and cardiac transplant patients
• Collaborate with medical director and referring physicians to ensure optimal patient outcomes
• Provide education and support to patients and families
• Manage patient records and data
• Develop and implement care plansRequirements
  • Education to meet certification, license or registration requirement
  • Oregon Registered Nurse License upon hire
  • VAD coordinator experiencePreferred Qualifications
