With a strong commitment to delivering individualized care, this organization empowers clinicians to make a real difference in the lives of patients-providing support, mobility, and independence in the comfort of their homes.As a Physical Therapist, you'll be responsible for evaluating physical conditions, creating personalized treatment plans, and guiding patients through their recovery journeys. You'll collaborate with a dedicated interdisciplinary care team to ensure every patient receives holistic, high-quality therapy services.Pay Range: $98,000-$108,000 per year (dependent on experience)Benefits Include:

  • Evaluate and assess patients' physical health and functional limitations
  • Design and implement customized physical therapy plans to reduce pain and enhance mobility
  • Educate patients and caregivers on therapeutic techniques, safety practices, and wellness strategies
  • Document patient progress and clinical updates in a timely and accurate manner
  • Collaborate with a multidisciplinary care team to deliver coordinated and patient-centered careQualifications
    • Active Physical Therapist license in the state of Oregon
    • Doctorate or Master's degree in Physical Therapy
    • Minimum of 1 year of experience as a Physical Therapist preferred (home health experience a plus)
    • Current CPR certification
