Facilities: Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and Saint Vincent *Note for Seaside floating only If your traveler is booked to a non-Seaside facility and asked to float to Seaside, they will receive $5/hr incentive for hours worked at Seaside in addition to IRS Mileage reimbursement. If floating consecutive shifts, they will be additionally offered a hotel. Facility agrees to give as much notice as possible when floating to Seaside. Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD) MUST request all RTO / specific scheduling requests up-front during submittal. Post submittal/offer RTO may not be considered or granted. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency. Modules: Modules are non-billable, average 16 hours spent.
