RN - MDS Coordinator at Providence Health and Services summary:
Registered Nurse (MDS Coordinator) responsible for coordinating
completion of the Minimum Data Set (MDS) assessments and resident
care plans for assigned long-term care residents, ensuring
interdisciplinary team participation and RN oversight of care
quality. Requires an active Oregon RN license, at least one year of
long-term care nursing experience, and prior MDS completion
experience; full-time day shift in an on-site cottage/child center
setting. Position is within a not-for-profit health system offering

Description RN - MDS Coordinator with the Child Center at
Providence Portland Medical Center, in Portland, Oregon. This is a
full time, day shift position Monday-Friday. Providence offers a

or coaching sessions per eligible member per year The MDS RN
coordinates the completion of the Resident Assessment Instrument
(MDS) and Resident Plan of Care for an assigned group of
residents/cottages. Tasks include ensuring participation of the
interdisciplinary team, resident and family as appropriate. This
position provides RN oversight as Care Coordinator to ensure
quality of care, quality of life and standards of practice. The MDS
Nurse promotes a resident-directed care model in the cottage
setting and utilizes all aspects of the environment and care model
to promote excellent care in a home setting to address the needs of

Graduate of an accredited school as a Registered Nurse. Upon hire:
Oregon Registered Nurse License. 1 year Nursing experience in
