Care Manager RN, Full-Time Day Shift at Providence Health and
Services summary: Registered Nurse Care Manager in an acute care
hospital provides discharge planning, transition coordination, care
planning, patient education, community referrals, and ongoing
monitoring of care plans to facilitate progression through the
continuum of care. Position requires RN license (Oregon), nursing
degree (BSN required within 3 years), acute care experience, EMR
proficiency, and care management or related experience; CCM
certification preferred. Full-time day-shift role at Providence
Portland Medical Center with benefits and potential for variable
shifts, on-site work, and a pay range of $57.90–$84.50/hr.

provide a variety of services to appropriately help progress
patients through the continuum of care. Key services include
screening for post-acute needs; transition planning; care
coordination; education; community care referrals; care planning in
accordance with regulatory requirements; collaboration with the
continuum care team; ongoing monitoring of plan effectiveness and
reassessment of the need for Care Management (CM) services.

Portland Medical Center Required Qualifications: Nursing
degree/diploma required upon hire. Bachelor's degree in Nursing or
higher within 3 years of hire. Upon hire: Oregon Registered Nurse
License 2 years acute care hospital or community based experience.
1 year experience with electronic medical records or documentation
systems. 1 year home health, mental health, substance use, hospice,
utilization review, discharge planning or care management
experience Preferred Qualifications: Bachelor's degree in nursing

for this position. The successful candidate's placement on the wage
range for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
