Physical Therapist Location: Salem, OR Agency: Cynet Health Pay: $2,718 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/9/2026 About the Position Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 year of adult home health experience License: Must have a valid state license in hand Certifications: CPR, BLS Must-Have: Must have reliable transportation. Must have a current driver's license. Must have agency-required liability insurance. Description: Home visits are required for this position. Periodic office meetings will take place. An average of 5-6 patients is expected per day. Contractor will be assigned between 5-10 patients daily, which equates to roughly 6-8 points. There is no guaranteed hourly minimum. Traveling up to 75 miles from the home clinic is required. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. Weekend requirements vary based on location and profession. There will be no on-call requirements. All documentation must be completed within 24 hours of the patient appointment. Clinician must accept every patient assigned, regardless of distance from previous appointments. Failure to comply may result in cancellation of the contract. No guarantee of hours is provided for this contract. Weekly productivity expectations are 30 points, equating to 40 hours of work per week. Travelers are subject to floating to nearby clinics if low patient census impacts financial needs. Experience with home health is required. Proficiency in HomeCare HomeBase EMR is necessary.
