Licensed Social Worker Location: Albany, OR Agency: Thrive Staffing Pay: $1,872 to $2,066 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title OR - Hospice/Outpatient Bereavement Coordinator - .6 FTE, Allied, Social Worker City/State: Albany, Oregon Shift: Days (08:00-16:30) occasional weekends, 8 Hour Days Contract Length: 8.00-5,91 Start Date: 01/29/2026 Job Description: The Hospice teams support patients and family members through the final stages of their illness, offering services in the home setting. These services include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support. Responsibilities: Assist patients and family members through the final stages of illness with counseling and support. Provide bereavement support as part of a comprehensive hospice care plan. Requirements: Bachelor's degree in social services, counseling, or psychology-related area required; Master's degree preferred. Healthcare Provider Level BLS required. Experience or education in grief or loss counseling required. Valid driver's license and auto liability/property damage insurance required. Experience in hospice preferred; additional experience in a medical setting preferred. Requirements Required for Onboarding Core Mandatory Part III
