Job Description:
Legacy Health is currently seeking a dedicated Clinical Social
Worker Specialist to join our innovative team committed to
improving the mental and emotional well-being of our community. As
part of our non-profit health system, this role will be integral in
providing exceptional patient-centered care across our extensive
network of hospitals and care facilities in Oregon and Washington.
The Clinical Social Worker Specialist will assess, diagnose, and
treat mental, emotional, and behavioral disorders through both
individual and group therapy sessions. Our collaborative
environment supports continuous professional development and a
multidisciplinary approach to health and wellness. Conduct
comprehensive patient assessments to determine psychosocial and
clinical needs. Develop and implement tailored treatment plans
based on assessments. Facilitate individual and group therapy
sessions. Collaborate with healthcare providers to integrate mental
health care with overall health strategies. Maintain detailed
records of patient progress and treatment. Advocate for patient
access to community resources and services. Continuously evaluate
and adapt treatment strategies as needed. Strong interpersonal and
communication skills. Expertise in psychological theory and
practice. Ability to handle high-stress situations calmly and
effectively. Proficiency in documenting and record-keeping with
various healthcare IT systems. Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Licensed Clinical Social
Worker (LCSW) or equivalent state licensure. Certification in
clinical social work or a related specialty is preferred.
