Job Description:
Legacy Health is seeking a dedicated and skilled Advanced
Practitioner/Clinical Nurse Specialist (CNS) to join our expanding
healthcare team. Our ideal candidate will bring a wealth of
knowledge and expertise to our patient care environment, supporting
our mission to provide compassionate and comprehensive care across
our network. This position entails both clinical practice and
education, focusing on enhancing patient outcomes, optimizing
medication management, and leading evidence-based projects. As part
of our team, you will collaborate with multidisciplinary teams
across our hospitals and outpatient facilities in Oregon and
Washington, utilizing your specialized skills to influence care
standards and practices. Conduct comprehensive and episodic health
assessments including physical examinations and interpreting
diagnostic tests Develop and implement disease management programs
and clinical practice guidelines Collaborate with physicians and
other healthcare professionals to deliver efficient and effective
patient-centered care Provide leadership in advancing
evidence-based nursing practices Educate patients and their
families on disease processes, treatments, and health maintenance
Mentor nursing staff and help facilitate their professional
development Advanced knowledge of disease management and nursing
practice in a specialized field Ability to perform comprehensive
health assessments and make evidence-based decisions Strong
leadership skills and ability to work collaboratively within
interdisciplinary teams Excellent communication skills and a strong
focus on patient education Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Current RN license and
Clinical Nurse Specialist certification in the state of practice
Authorized to prescribe medications (if applicable in state of
practice)
