Job Description:
Legacy Health, a leading non-profit health system renowned for

Nurse Practitioner to join our dynamic team in the Outpatient
Specialty Locations across Oregon and Washington. This role focuses
on providing high-quality patient care in specialized outpatient
settings. Our Nurse Practitioners play a crucial role in our
mission to improve the health and well-being of our communities,
managing patient care in a collaborative environment, and focusing
on quality outcomes. Conduct comprehensive or episodic health
assessments of patients with acute or chronic health problems in
order to establish medical diagnoses. Manage patient care in a
collaborative effort with other care providers and specialists.
Prescribe medication and treatment plans while managing therapeutic
interventions. Promote health and disease prevention within the
community through education and intervention strategies. Monitor
the effectiveness of interventions and adjust treatment as
necessary. Document patient care services by charting in patient
and department records. Advanced diagnostic skills with a focus on
accuracy. Strong interpersonal and communication skills. Ability to
manage intense and crisis situations efficiently. Proficient in
using medical software for patient care management. Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Current Nurse Practitioner licensure in the state of Oregon or
Washington. Board Certification from the American Nurses
Credentialing Center (ANCC) or equivalent.
