Job Details - Location: Portland, OR 97220 and surrounding areas (up to 75-mile radius) — Home Health RN jobs Portland - Assignment Type: Travel RN, 13 weeks - Shift: Day shifts, Monday–Friday, 8:00 AM – 5:00 PM - Hours per Week: 40 - Productivity: 30 points per week (typically 5–10 patients per day) - Mileage: Reimbursed at IRS standard rate (avg. 20–40 miles/week) - Estimated Weekly Salary: $2,050–$2,160 per week - Start Date: March 23, 2026 - End Date: June 22, 2026 Required Qualifications - Active Registered Nurse (RN) license in Oregon (must have license in hand) - Minimum 1 year recent Adult Home Health or Hospice RN experience - Current BLS and CPR certifications - Experience with HomeCare HomeBase (HCHB) EMR and OASIS (used within last 6 months or willingness to retrain) - Reliable transportation, valid driver’s license, and auto insurance - Willingness to travel up to 75 miles from the home clinic - Ability to participate in on-call rotations (2–4 times per month) - Strong clinical assessment, documentation, care coordination, and time management skills Key Responsibilities - Provide skilled nursing care in patients’ homes according to physician orders and agency policies (Home Health RN responsibilities) - Perform comprehensive assessments, admissions, OASIS documentation, wound care, medication management, and patient education - Coordinate care and communicate effectively with interdisciplinary team members, patients, and families - Supervise and evaluate Home Health Aides and LPNs/LVNs as needed - Complete all documentation within 24 hours of visits using HCHB EMR - Travel between patient homes within the assigned service area and attend periodic office meetings - Participate in on-call and occasional weekend rotations as required - Maintain infection control standards and ensure patient safety during home visits Experience the best of Portland while advancing your nursing career in a supportive and rewarding environment.
