Overview

This position works in collaboration with physicians to provide complete physical examinations, diagnosis, and treatment of both chronic and episodic disorders. Assigned principal responsibility for primary health care of patients. Seeks the expert opinion or advice of a physician whenever the incumbent decides a case falls outside the scope authorized by policies and protocol. The APP typically remains responsible for the patient throughout the course of treatment, provides the physical examination, the diagnosis, and the health care plan, determines when the patient has recovered, and releases the patient. The APP may refer the patient to a physician for consultation or to specialized health resources for treatment, but retains responsibility for clinical management as well as any subsequent modification.

Responsibilities

Duties of Advanced Practitioner - Wound Care

    • Provide initial and follow-up evaluations of patients with chronic wounds in an outpatient and inpatient setting
    • Perform wound debridement to encourage wound healing
    • Coordinate care with social workers, home health, and case managers
    • Place total contact casts for offloading of neuropathic foot ulcers
    • Manage the use of advanced wound care options such as Negative Pressure Wound Therapy, Cellular Tissue Products, Hyperbaric Oxygen Therapy
    • Work in a multi-disciplinary clinic with occupational therapists, lymphedema therapists, burn surgery, and podiatry
    • Outreach with other Legacy Health wound clinics
    • Active participation in department leadership meetings for program development
    • Active participation in patient care conferences to improve patient outcomes

      Qualifications

      Education: As required by licensure. Experience: Clinical experience requirements are dependent upon the specific position, and include a minimum of 2 years of experience in a related field.
