Job Description:

The RN participates as a member of the health care team in delivering quality health care to patients, which supports KP's mission, vision & values. The RN provides a variety of nursing srvs both in person & by phone utilizing the nursing process & leadership skills to address acute & chronic needs of Health Plan members & other patients of the KP Health Care Program. He/she works under the general direction of the designated Sup-v.

Essential Functions:

Directs, delegates & monitors quality of care delivered by the health care team & brings deviations to the attention of staff involved & Team Leader. Mentors new staff & assists w/ orientation & staff development needs. Collaborates w/ Team Leader & Mrg in setting quality standards for nursing care & in the development of policies & procedures. Provides input to Team Leader & Mrg about the job performance of other RNs, LPNs & unlicensed personnel in the health care team. Demonstrates commitment to customer srv.
- Participates in population-based care activities. Participates in out reach & in reach activities to meet clinical targets. Effectively identifies needs & provides prevention, early intervention & ongoing health education for panel population, individual patient & family. Assists individuals & groups to learn self Mgmt skills that promote & protect health. Develops teaching plans for individuals & groups related to health promotion & disease prevention.
- Demonstrates technological literacy in using computerized info systems. Accesses, evaluates & uses data & info from a wide range of resources. Demonstrates effective use of computer-based info & practice Mgmt systems for decision support, provision of care, continuous quality improvement & professional development. Provides input to the development of info systems that impact the provision of patient care. Facilitates the members' use of health care technologies.
- Participates in ongoing continuing education activities & shares knowledge w/ other health care team members. Demonstrates commitment to continuous learning that advances professional nursing practice. Demonstrates ability to identify own learning needs. Actively works to address personal & professional growth needs. Serves as resource, mentor & role model for other members of the health care team.
- Clinical Competencies: Provides evidence-based, clinically competent nursing care in a culturally sensitive manner. Plans, implements, manages, coordinates & evaluates a plan of care across the care continuum. Facilitates member wellness & function & identifies needs for health care interventions. Utilizes appropriate assessment tools, processes, clinical pathways & guidelines to manage patient care. Integrates theory & research-based knowledge in the provision of direct & indirect nursing care. Performs & modifies therapeutic & preventive nursing measures & administers treatments & medications as authorized by law & determined by the State Board of Nursing. Performs skilled nursing tasks competently & demonstrates operational knowledge of procedures & equipment. Performs triage & telephone advice independently & w/in scope of the Oregon &/or Washington Nurse Practice Acts. Maintains a safe & therapeutic environment for staff, patients & family members. Participates in the research process. Documents the nursing care provided which demonstrates the application of the nursing process & complies w/ Deptal &/or organizational standards.
- Applies effective oral & written communication skills in interacting w/ patients, families & members of the health care team. This includes effective negotiation & conflict Mgmt skills.
- Organizational Competencies: Recognizes the impact of systems on health care delivery. Facilitates coordination of care across all settings w/ particular attention to transitional needs. Functions w/in interdisciplinary teams in a manner that promotes coordination, mutual respect & timely response to the patient's health care needs.
- Acts as an advocate for individuals & groups w/ unmet health needs
- Applies the knowledge & tools of continuous improvement in clinical practice & the delivery of health care.
Basic Qualifications:
Experience
- Minimum two (2) years of experience in acute care or ambulatory care/clinic setting within the last 5 years.
Education
- Graduate of Accredited School of Nursing required.
- High School Diploma or General Education Development (GED) required.
License, Certification, Registration
- Current Oregon AND Washington RN license required. License in state where assigned required upon hire/transfer, application for license in secondary state required within 2 weeks of hire/transfer and must be obtained within 6 months.
- Basic Life Support (BLS) for Health Care Providers required upon hire/transfer.

Additional Requirements:
- Demonstrates clinical nursing and leadership skills.
- Able to type 20 words per minute.
- Demonstrates effective interpersonal, communication and problem-solving skills.
- Willingness to learn computerized information systems.
- Demonstrates ability to work within teams and within a dynamic work environment.
- Customer focused.
- Final candidate must pass pre-employment language skill assessment administered over the phone by Kaiser Permanente.
- Upon hire, must take the next available Kaiser Permanente Qualified Bilingual Staff (QBS) training for continued employment.

Preferred Qualifications:
- Current or future bachelor's degree in nursing or related field.
