Job Description:
Legacy Health, a leading non-profit health system renowned for
compassionate care, is seeking a dedicated Clinical Social Worker
to join our dynamic team. With facilities spanning across Oregon

Social Worker, you will play a crucial role in addressing the
psychosocial aspects of healthcare, providing counseling and
support to patients and their families, helping navigate the
complexities of health care systems, and contributing to patient
care plans. Conduct assessments of patients' social, emotional,
financial, and family situations as part of interdisciplinary
healthcare teams. Develop and implement care plans in coordination
with medical professionals, addressing patient-specific
psychosocial needs. Offer counseling and crisis intervention
services to patients and families. Facilitate patient access to
community resources and services, including setting up home care,
rehabilitation, and follow-up appointments. Advocate for patients'
rights and necessary accommodations in treatment plans. Maintain
comprehensive patient records and participate in staff training and
professional development opportunities. Strong interpersonal and
communication skills Ability to manage high stress situations
Proficient in patient assessment and counseling Knowledge of
healthcare systems and community resources Expertise in
documentation and patient record management Medical Insurance

Clinical Social Worker (LCSW) in the state of practice Optional
certifications that could be beneficial include Certified Advanced
Social Work Case Manager (C-ASWCM) or Certified Social Work Case
